Profad Care Agency believes that palliative and end of life care should be person centred and the needs and wishes of the individual should be recognised and acted upon. We will initiate discussions as early as possible to establish the needs and wishes of the Individual such as where and how they would like to die and any culturally appropriate methods which may be required.
Profad Care Agency ensures we communicate effectively with the individual, families and friends as well as any other advocates involved in the individuals care and support. We will provide support for carers and families during this difficult time including during the illness of their loved ones and after their death. The individual’s carer/s, families and friends have a vital role in the care and support delivery with palliative and end of life care, and we will ensure they are fully supported during this time.
Profad Care Agency believes the best service for end of life care is delivered through a holistic approach. We believe this is achieved through working in partnership with the individual, the carer/s, Family as well as other healthcare professionals and voluntary and community sector groups, to ensure service quality promotes independence, choice and promotes social interaction.
